Accident Investigations - Learning from Mistakes
Assistance is given to members to complete the F2508 form to be sent to the HSE within
10 days of the incident or dangerous occurrence. The Worksafe Partnership support its
members as required either in accident investigations or litigations when dealing with
the enforcing authorities. (We will also provide HSE/HMSO stationary at cost if required to do so).
All accidents investigated on behalf of members will be undertaken without delay, together with an
immediate interim report back to the Managing Director, Propretor or Safety Director.
We can represent you when dealing with the HSE or other investigating officers i.e. from the Client,
your Insurers, or Principal Contractor Safety Officers.
All investigations will comply with RIDDOR (The Reporting of Injuries, Diseases & Dangerous
Occurrences Regulations 1995) requirements and will meet insurer's expectations including witness
statements, photographic records and accident records, all assembled in one report together with
any recommendations to prevent re-occurrence as required by the regulations.
